Error when trying to create rounded edge

I was following this tutorial:

It mostly worked fine. All the corners of the box I’m creating came out perfectly, except the one where I create the curve. It just leaves this weird hole in the geometry.

This is what happens when I attempt the same technique:

My box is quite shallow. 150 * 130 * 2 millimetres.

You are working too small, have a look at this thread.
