Is it possible for an extension to register a custom model info or preferences page?
No.
Model info are not extensible, but model attributes are and you can use an existing attribute viewer or built a GUI for your attributes.
Gathering plugin preferences in SketchUp’s preferences has been suggestest (and sadly still does not exist), so currently a 1-button-plugin would have to add a second button and a dialog to modify its settings. Web browsers and IDEs are good examples where individual plugin settings are organised as buttons in the plugin manager and not scattered everywhere.
