Maybe this approach would help. This is a future extension, LordOfTheToolbars (LOTT) which I am trying to finish.
It includes a small toolbar (Vignette Stripe) from which you can access configurable palettes of icons taken from Ruby toolbars. There is also an history of ommands, custom toolbars and a few other features.
