Why does it make a difference which direction I use the follow me tool to get faces oriented correctly?
I believe it depends on the front/back orientation of the profile relative to the first segment of the path. If as you look down the first segment, you see the back face of the profile, the extrusion will have back faces out. If you see the front face, the extrusion will have front faces out.
